Guests looking for slightly more upmarket accommodations should consider Terme Manzi Hotel & Spa, which has a contemporary spa and ornate rooms, though rates are higher. 

Poggio Aragosta Hotel & Spa

The family-run, 50-room Hotel Poggio Aragosta is an upper-middle-range property with wonderful gardens and lovely sea views on Italy's Ischia Island. Homey and simple Mediterranean-style rooms have vibrant tile floors, balconies or terraces, and delightful ocean or garden views. The many couples that stay here frequent the two rooftop pools, one of which is heated by the island's famous thermal springs. The hotel has a small spa with an indoor pool, a modestly appointed fitness center, and on-site restaurants and bars plus a free buffet breakfast. A free area shuttle is also offered. Guests looking for a more chic island getaway may want to consider the Garden & Villas Resort, though rates are significantly higher.

The 70-room Thermal Park Nausicaa Palace Hotel is a three-pearl property near Casamicciola Terme on Ischia Island's north shore. The name may conjure images of grandeur and bubbling thermal baths, but that's only half true. The hotel and its guest rooms have outdated decor, furniture, and amenities (including tube TVs in rooms) and the on-site spa, while providing a range of treatments, is austere and resembles a hospital. The main draw here is the pleasant outdoor thermal park with several pools and hot tubs heated by the island's famous hot springs. There is a fitness center on site as well as a restaurant that serves a continental buffet breakfast and a bar, though there's nothing lively or fresh to be had on offer here. 

Gran Paradiso Hotel

The 46-room Gran Paradiso Hotel is an upper-middle-range property with sea and mountain views in the small town of Casamicciola Terme on Ischia Island's north coast. Classic, cheerful rooms have tall windows and traditional tilework, and most have balconies or terraces with lovely sea or mountain views. However, furniture is a bit worn and the style is dated (like many properties across north Ischia). Thermal and fango mud treatments are available at the modern on-site spa that's a step above many hotels in this area, as are the indoor and outdoor pools. The on-site restaurant and free breakfast receive mostly good reviews, and there are no fees for Wi-Fi or limited on-site parking. Guests looking for a more stylish place to rest may want to consider the Garden & Villas Resort, though rates will be higher.

The family-run, 26-room Hotel Pensione Monti is a homey, warm, and welcoming mid-range property with fantastic sea views that’s within a 15-minute walk of the shops and restaurants in the lively little town of Lacco Ameno. Basic rooms are comfortable and similar to many pensiones (small guest houses) across Italy, with neutral color schemes and a slightly dated style (a quality that defines most of the hotel). Air-conditioning is available by request only and requires a daily fee. The on-site restaurant plus the spa and outdoor thermal pool are major highlights for guests, and there is a lovely garden as well as outdoor terraces with beautiful views over the Mediterranean.

Hotel Villa Janto

The intimate 16-room Hotel Villa Janto is a mid-range hotel with a homey atmosphere and lemon trees dotting its compact grounds. Nestled in the residential hills outside of Lacco Ameno near Ischia's northern coast, the hotel runs a free shuttle into town, which is otherwise a 25-minute walk. Simple, bright, and pleasant rooms are spotlessly clean and have small balconies -- they are far nicer than many throughout Ischia. With a thermal pool, a thermal sauna, and an outdoor pool, plus a bar and an exceptional on-site restaurant that uses produce from the owner’s garden, the warmth and charm here make this one of the better values in this corner of the island.

Grifo Hotel Charme & SPA

Grifo Hotel Charme & Spa is a mid-range, 30-room property in Casamicciola, a spa town in the island of Ischia, in the Gulf of Naples -- though it's built closer to a pine forest and private homes than the sea. The hotel provides a free shuttle service to Castiglione and Bagnitiello spa parks and has a sophisticated outdoor pool with stunning views of the nearby hillsides. Many guests take advantage of the wellness spa in the basement. An on-site restaurant serves free breakfast buffet, a la carte lunch, and a dinner buffet with Mediterranean ingredients. Rooms are clean and unforgettable in decor, but many have beautiful views from private terraces or balconies. An early check-out time can put a damper on romantic and relaxing holidays. 

Terme Manzi Hotel & Spa

Ischia's historic four-pearl Terme Manzi Hotel & Spa dates back to 1863, when it was constructed around the Il Gurgitello hot spring that feeds its thermal pools to this day. In 2011 the hotel was thoroughly renovated and today its 58 rooms have an eclectic mix of classic and contemporary decor. There's an on-site Michelin-rated restaurant and free, though limited, shuttle to a local beach. However, the hotel is located in a congested residential area without much going on. The similarly priced Mezzatorre has a better location, in a green park with views of the sea. 

Hotel Terme Cristallo Palace

The 121-room Hotel Terme Cristallo Place is a mid-range option located on Ischia's north coast, in the charming town of Casamicciola Terme. Simple, Mediterranean-style rooms have ceramic tile throughout, with flat-screen TVs and air-conditioning, and some have shared terraces with amazing sea views. Rooms are a bit dated, though, a common trait in many hotels throughout Ischia. The main draw here are the several outdoor and indoor pools, many of which are heated by the island's famous hot springs, and the on-site spa is one of the better ones that we saw on Ischia. There is also an on-site restaurant and bar. Fees for extras here can add up, including those charged for Wi-Fi, steam room use, and on-site parking.
